We encourage Parents/Guardians who are interested in having their child/ward enrolled at Southborough Primary to kindly use the online registration form. Before starting your online application, please ensure you select the correct form and you have all the supporting documents on hand.

Please note your form will not be submitted if:

 1. Required fields are left blank

 2. Address is not filled out completely

 3. Your contact email address is missing or incorrect

 4. The form has been left idle for too long 

Please pay close attention to the list of supporting documents and the guidelines outlined. Failure to submit ALL supporting documents or adhering to the guidelines outline may result in your child/ward's application not been processed.   

Passport Size Picture - A recent passport size picture not older than six months must be uploaded. No baby picture will be accepted.

Copy of Birth Certificate - Only  a copy of the original birth certificate will be accepted, that is the copy issued by the RGD.

Copy of Immunization Passport  - Please ensure pages  8, 9, 14 and 15 are uploaded. Please ensure that the required fields on pages 8 and 9 are filled in and the required shots and boosters for entrance to primary school are received.
